Transition services and supports will be developed to address the needs of the individual and their family. As part of a support network the Transition Facilitator assists with transition supports. These innovative supports will enhance the development of the necessary skills and resources to facilitate the successful transition from school or home to community living (which may include but are not limited to: work, volunteering, day program, social and recreational activities, further education, independent living etc.).
All services and supports provided will be respectful of the social, linguistic and the cultural diversity of families and communities and follow the QAM support services guidelines.
