Supported Independent Living
This program is designed to provide individually tailored residential supports to assist adults with a developmental disability to meet their goal of living independently (or inter-dependently) in the community. Facilitators provide assistance in various areas such as meal preparation, personal hygiene, medication administration, budgeting, use of public transit, home management, health promotion and community orientation and participation. Facilitators also support individuals in their choice of working, volunteering or leisure activities. Supports are based on the principles of person directed planning, self-determination, social inclusion, citizenship and choice.
Transition Facilitation
Transition services and supports will be developed to address the needs of the individual and their family. As part of a support network the Transition Facilitator assists with transition supports. These innovative supports will enhance the development of the necessary skills and resources to facilitate the successful transition from school or home to community living (which may include but are not limited to: work, volunteering, day program, social and recreational activities, further education, independent living etc.).
Community Connections
This program is designed to promote independence and personal development in the areas of basic life skills, work, social and recreational activities, community participation/utilization and citizenship for adults with a developmental disability who are at least 18 years of age.

Madoc C.O.P.E. is able to serve the unique needs of you and your family. We can tailor our fee-for-service opportunities based on your needs. Our agency complies with the Ministry of Children Community and Social Services.

Associate Home Program
This program's objectives and goals are to provide accommodation, care, and supervision to adults with a developmental disability in an associate home. Services and Supports provided to promote social inclusion, individual choice, independence, and rights.
